Policies

New Patient Appointments:
Please arrive 15 minutes before your scheduled new patient appointment time. This will allow us to obtain your ID, insurance card, and will also allow appropriate time to complete new patient paperwork.

Missed/Cancelled Appointments:
We must be notified of rescheduled or cancelled appointments at least 24 hours in advance of the scheduled appointment time. A $25.00 fee will be directly billed to all patients who do not provide notification within that time frame.

Missed/Cancelled Surgery:
A significant amount of time is placed into preoperative planning. This involves a team approach including imaging review, hospital staff, your primary care physician, our OR scheduling team, OR staff, as well as surgical representatives to ensure appropriate equipment and implants are prepared for your case. We put forth significant effort to ensure your entire surgical experience runs smoothly, including discussion of insurance coverage, deductibles, and hospital costs. We must be notified of rescheduled or cancelled surgery at least one week prior to the surgery date. A $300.00 surgery cancellation fee will be directly billed to all patients who do not provide notification of surgery cancellation within that time frame.
